  • The Level Funded Freedom Plan when attached to a high deductible health plan (HDHP) meets ERISA requirements. This plan offers a zero deductible, 50% co-insurance, first-dollar coverage and individual employee maximum out-of-pocket of $3675. Employers receive back 95% of their unused claim funds.
